長期照護學實習(一)

計畫個案照護之護理過程及成果評值,並將護理實證資料應用於個案照護,除此之外,學生需整合所修習專業知識、技能及能力進行專案規畫,透過跨專業的團隊合作,以達到促進個案健康、改善個案生活品質。 Exploration of nursing intervention classification and the nursing outcomes classification related to the care of the clients; illustrating how nursing practice data (diagnoses, intervention and outcomes) can be used at the individual, particularly in determining the effectiveness of nursing intervention. In addition, the student can integrate the knowledge, skill and competency from other discipline and to work collaboratively within a multidisciplinary team.

Establish nursing intervention data base including knowledge and understanding relationship between another person and resident environment. 2. Provide comprehensive health and social care management for disabled persons in collaboration with other health care professionals and agencies. 3. Provide comprehensive health care management for clients in collaboration with other health care professionals and agencies. 4. Provide transitional care by using community resources and establishing long-term care support network for residents. 5. Generate hypotheses requiring further testing in relation to the nursing care of clients whose presenting concerns are in the physiological and psycho-social aspects. 6. Understand the function and service of the day care, and find the potential development in it.
