長期照護學實習(二)

After complete the class, the student can understand the role and function of care manager at long-term care association; manage the long-term care of individual; test care plan for dealing with health and social problems; assess community resource to provide continuing care; recognize indicators to evaluate quality of care for long-term care facilities. Except LTC management center, the students would have clinical practing in Day Care and Home Care, let them understand multiple LTC services. In addition, the student can evaluate the strength and weakness of long-term care system, and address possible strategies for stimulating long-term care resources.

Provide comprehensive health and social care management for disabled persons in collaboration with other health care professionals and agencies. 提供失能者照護計畫並整合跨團隊服務 4. Provide transitional care by using community resources and establishing long-term care support network for disabled persons. A單位社區資源整合,建立長期照顧網絡 5. Analyze hindrance of care delivery and synthesize possible strategies to construct seamless care delivery system 分析長照服務供給的策略及建構無縫接軌的服務系統
