Through participation in the course the student will be able: 1. To collaborate with residents, community members, gate keepers of cases/target group, professionals and other community groups in the process of community/aggregate health program planning, implementation and evaluation; 2. To match multi-disciplinary resources to help students develop innovative community health promotion or case management program. 3. To develop a plan and write a program proposal with community input for amelioration of the problem through a. establishment of goals b. determination of possible intervention strategies c. specification of measurable objectives d. generation of strategies for achieving the goals and objectives e. selection of appropriate actions to meet goals and objectives f. design of an evaluation for the planned intervention; 4. To implement appropriate phases of the planned intervention toward the target groups in the community; 5. To analyze the effect of organizational structure and process in public health practice; 6. To analyze the interacting constituents of the sociopolitical environment 7. To analyze problems and issues involved in implementing and evaluating community/aggregate health promotion programs; 8. To plan with community inputs for continuity of planned intervention and evaluation; 9. To synthesize theory and practice of community health promotion and/or case management in primary health care. 10. To provide business operation concepts to assist students to develop feasible community health program that take into account quality and cost; and encourage business verification.
This course covers the theoretical base and process of development, planning, implementation, and evaluation of community-based health promotion or case management programs. Channels to reach the community target group and/or identify cases will be established. Collaboration and planning with community groups and/or gatekeepers of case aggregates as well as developing evaluation strategies will be emphasized. Importance of system, coordination, referral, health information system will be emphasized in community health planning. Incorporation of cost effectiveness, economic development, business model, innovations, and operational concepts in community health planning will be facilitated. The students could base their planning of health promotion or case management programs on health diagnosis from the prior course Community Health Assessment and Planning. In collaborations with community residents, consumers, health care providers, and other stakeholders, students develop innovative approaches directed toward amelioration of a health problem identified in a community or community aggregate. A community health or case management plan, including innovative strategies and an evaluation, is designed and the plan is implemented. This course incorporates instruction, seminars and community experiences.
