智慧整合式照護模式設計與運用

本課程的目的會先從整合,系統與智慧的觀點來介紹健康管理平台設計的概念,然後帶領大家從動手做實際感受到數位化平台的產生過程,讓大家思考從傳統紙本思維到數位化的改變,調站所謂整合式評估的設計與未來.其中也將介紹個人與團隊長久研發在高雄市首先推行的御老居家整合平台成果,和各位分享健康管理模式的挑戰與機會.  課程目標如下: 1. 了解何謂整合式照顧?模式的關鍵內容? 2. 了解推行整合是照顧的價值與瓶頸 3. 透過實際案例了解整合式照顧工具的開發與使用 4. 了解健康資訊學的機會與挑戰 5. 了解最新.陽明校友開發國際領先,並於高雄市全面推廣的御老居家養老平台 6. 針對自己有興趣的專題形成計畫書
