長期照護學實習(一)

Exploration of nursing intervention classification and the nursing outcomes classification related to the care of the clients; illustrating how nursing practice data (diagnoses, intervention and outcomes) can be used at the individual, particularly in determining the effectiveness of nursing intervention. In addition, the student can integrate the knowledge, skill and competency from other discipline and to work collaboratively within a multidisciplinary team. 1. Establish nursing intervention data base including knowledge and understanding relationship between another person and resident environment. 2. Provide comprehensive health and social care management for disabled persons in collaboration with other health care professionals and agencies. 3. Provide comprehensive health care management for clients in collaboration with other health care professionals and agencies. 4. Provide transitional care by using community resources and establishing long-term care support network for residents. 5. Generate hypotheses requiring further testing in relation to the nursing care of clients whose presenting concerns are in the physiological and psycho-social aspects. 6. Understand the function and service of the day care, and find the potential development in it.
