Course Description The purpose of this course is to enhance advanced practice nurses capability to improve individual, family and population health through using both humanistic and evidence based translation concept and theory and provide best health plan. In addition, students will be able to develop and lead the plan based on results from individual, family, community health assessment, critical and systemic thinking gap analysis, and evidence based versus evidence translation utilization. Course Objectives 1. To discover individual- and population-based health perspectives 2. Identify social determinants of health and describe the impact of social determinants on health 3.Using critical and systematic thinking to conduct gap analysis and compare, describe, and monitor the health of populations. 4. Using evidence based and evidence translation concept and theory to develop best health plan. 5. Utilizing population and community assessment results to develop evidence-based, health promotion and disease prevention interventions in the clinical setting or community. 7. Develop strategies based on practice inquiry and translation evidence skills to minimize the gap between best practice and clinical practice as such, improve quality of care.
