長期照護學實習(一)

Exploration of nursing intervention classification and the nursing outcomes classification related to the care of the clients; illustrating how nursing practice data (diagnoses, intervention and outcomes) can be used at the individual, particularly in determining the effectiveness of nursing intervention. In addition, the student can integrate the knowledge, skill and competency from other discipline and to work collaboratively within a multidisciplinary team. 1. 學生根據學理及評估個案與他人、環境間關係,建立適宜護理措施 Establish nursing intervention data base including knowledge and understanding relationship between another person and resident environment. 2. 學生透過跨專業合作,提供失能個案完整的健康照護管理 Provide comprehensive health and social care management for disabled persons in collaboration with other health care professionals and agencies. 3. 學生利用社區資源及長照網絡服務,提供轉銜照顧 Provide transitional care by using community resources and establishing long-term care support network for residents. 4. 學生應用護理理論提供個案身心靈方面的護理照護 Apply a theory to the clients via nursing care whose presenting concerns are in the physiological and psycho-social aspects. 5.學生發現機構潛在困難,嘗試提出可行之實證策略 Finding potential problems in the care facilities, and try to apply some workable strategies trough evidence-based researches.
