Course abstract:We review the recent progress on diagnostics and therapeutics based on extracellular vesicles (EVs, lipoproteins), supermeres and exomeres. These nanocarriers from diseased cells are low in abundance (102 to 106 per ml of blood) and are extremely heterogeneous with different subsets carrying distinct biomarker cargoes. The ligands and receptors on their surface can also direct them to specific cells, thus enabling non-immunogenic drug delivery with long retention time. We examine the key issues that have so far prevented regulatory approval of the nanocarrier diagnostics and therapeutics, after nearly two decades of intense research. We also suggest possible research directions that may overcome these challenges, including some from my lab that are now ready to be commercialized.
