概述

Course Description The specific objective of this course is to enable students to develop an innovative and systemic perspective of care to improve chronic illness prevention and management that is evidence-based, theory driven and client-centered. Course Objectives Upon completion of the course, the student will be able to: 1. Identify the complex behavior and psychosocial challenges for people living with chronic illness, their families, care givers, aggregates, and communities. 2. Review and discuss the evidence related to the nursing practice of a range of chronic illness. 3. Identify theoretical models for prevention and managements of chronic illness. 4. Examine the challenges and contextual issues within the cultural, social and health care environment for people living with chronic illness, their families, care givers, aggregates, and communities. 5. Put and discus innovative ideas of prevention and management for chronic illness. 6. Critically appraise the effective prevention and management of clients with chronic illness based on a systemic approach 7. Translate evidence knowledge and innovative ideas into practice in prevention and management of chronic illness (Proof of Concept, Proof of Service, and Proof of Business).

Prepared presentation in comprehensive profiles of preventing and managing selected chronic illness (30%) (1) The impacts and trend of the specific chronic illness (2) The profiles of theory-driven and systemic approach of the specific chronic illness (3) The gaps and chances of needs and unmet demands of the specific chronic illness (4) A management and/or prevention program designed with evidence for the specific chronic illness by a selected system model 4. Final written paper (30%) (1) Use the format required by the particular professional journal which you plan to submit your manuscript. (2) Attach a confirmation letter of submission from the selected journal is encouraged.
